Wellness Center Welcome Letter

2019-2020

Dear Students and Parents,

The Wellness Center provides a variety of services to students attending Lake Forest High School. Bayhealth Medical Center serves as the medical vendor for the Wellness Center, and has provided adolescent care through school based Wellness Centers for many years.

Treatment of minor illness/injury, athletic physicals, immunizations, counseling and nutrition are some of the services we offer. Our services supplement those provided by the school nurse and guidance office.

Highlights regarding membership are noted below:

  • New students will be required to have the 2019-2020 registration packet completed before services can be accessed. The packet includes a new consent, registration and health history forms . The forms are also available in the Main office, school web-site, Guidance office and School Nurse offices.

  • Students 18 years or older may sign for themselves to receive services. Parents or guardians of minor students (less than 18 years) should complete the new forms.

  • The Wellness Center submits visit claims to insurance companies for services provided. You may receive an explanation of benefits (EOB) if your student accesses Wellness Center services. Students or parents will not be expected to pay traditional co-pays for services.

  • In order for us to provide services it is necessary that all sections of the forms be complete and we receive the requested insurance information on the registration form. It is essential that you be aware of the terms and limitations of your insurance coverage and that you provide us with updated information, as needed.

The Wellness Center is normally open when school is in session from 7:00 a.m. to 3:30 p.m. If you have any questions about Wellness Center services you can contact us at 284-9291 Ext 627. Please return the completed registration packet to the Wellness Center.
