Dear Students and Parents:

The Wellness Center would like to welcome you to Lake Forest High School. The Wellness Center provides a variety of services to students attending Lake Forest High School. Bayhealth Medical Center serves as the medical vendor for the Wellness Center, and has provided adolescent care through school based Wellness Centers for many years.

At the Wellness Center We Provide:

·       Treatment of minor illness/ injury

·       Physicals

·       Immunizations

·       Mental Health Counseling

·       Nutrition

The Wellness Center is open when school is in session 7:00am to 2:30 pm. If you have any questions about the Wellness Center services, you can contact us at (302)284-9291 ext 627. Please return completed consent form to the Wellness Center.
