Students, parents/guardians and any member of the Lake Forest Community can anonymously report bullying or illegal activities related to Lake Forest School District schools by submitting a Reporting Form using one of the links below. Reports will be automatically forwarded to the Lake Forest School District Administration and the appropriate Building Administrator for investigation.

To report bullying, please select the building in which it is occurring:

• South Elementary

• Central Elementary

• W.T.Chipman middle

• LFHS

Though reports may be made anonymously, the more information provided in the report, the more helpful that report is. Should you wish to be contacted by District Administration please include your name and contact information in the appropriate fields.

The District will fully investigate all reports, anonymous or otherwise. However, no disciplinary action will be taken against an alleged perpetrator solely on the basis of an anonymous report.
