The BayHealth LFHS Wellness Center will be doing DIAA Sports Physicals for FREE on Monday, June 30th 2025. Please contact the Wellness Center at 302-284-9291 Ext. 627 . The best times to call would be on Monday June 16th or Monday June 23rd between 7-3:30, as someone will be in the office to talk with you. If you cannot call one of those days, please leave a detailed message with students name and DOB and a call back phone number. You will receive a call back with an appointment time. No walk-ins will be accepted.

UPDATE: The Athletic Trainer, Keely Arndt, is also holding a physical turn in night on July 9th as well from 4-6 in the pool lobby. Any student who wishes to use the Athletic facilities this summer must have a current DIAA sports physical turned in to Nurse Dean by June 10th. If you are unsure if you have a current one, please see her in the nurse’s office, or email her at allison.dean@lf.k12.de.us . It must be on the form linked below, and be dated on or after April 1, 2025. If you are a member of the Wellness Center, you can get on for FREE there during school hours, just stop by to make an appointment ASAP. Also, Nurse Dean will be hosting a DIAA Sports Physical Turn-in Night on Wednesday, July 30th from 4pm-6pm at Lake Forest High School in the pool lobby area (please enter using those doors). ALL Fall Athletes who wish to play a sport must have their physical turned in by this date in order to attend tryouts/pre-season. The physicals must be on the current DIAA form linked below with the 07/02/2024 date on the bottom of each form, and must be completed on or after 04/01/2025.
